Each home is designed to foster connection and stability, giving residents a sense of belonging while maintaining their individuality. Caregivers create personalized care plans, incorporating outside services like hospice or therapy as needed.
Our Adult Therapeutic Foster Care homes are catered to individuals with Serious Mental Illness (SMI), providing additional therapeutic support and guidance to help them transition toward greater independence.
Whether it’s short-term respite or long-term care, our goal is to match each resident with a compatible home where they can thrive in a safe and supportive setting.

Understanding Eligibility for Adult Foster Care
Clients must meet certain criteria to qualify for AllThrive 365’s Adult Foster Care Program:
- Must be 18 years or older.
- Title XIX (Medicaid/AHCCCS) eligible
*For Adult Foster Care Therapeutic Homes: Must have a Serious Mental Illness (SMI) diagnosis
Open Your Home as a Provider
Interested in opening your home as an Adult Foster Care Provider?
- 1-year caregiver experience preferred
- Valid caregiver certificate from an accredited program. See approved list here (hyperlink: www.nciaboard.az.gov)
- Resides in a single-level family home that can accommodate up to four residents
- Steady income from at least one household member
