AllThrive 365 empowers Mercy Care Advantage Members in Santa Cruz, Maricopa, and Pima counties with a personalized Wellness Program designed to improve health and prevent chronic disease.
Through one-on-one education and phone consultations, our Wellness Team provides real-life guidance tailored to each individual’s needs. Our program focuses on four key areas: Nutrition Education, Diabetes Prevention, Exercise Programs, and Smoking Cessation.
In addition, our Community Health Workers are embedded in FSL housing communities, offering direct support, health education, and resource connections to help residents achieve long-term well-being.
